Each relationship I build with a client begins with two meetings.

 

The first meeting will be an hour long, and in it I will answer your questions and ask several of my own, the goal being for us to determine if we are a good fit.

 

The second meeting will also last about an hour, and I will present you with a customized plan of care developed using information shared at our first meeting. At this time, you will decide to move ahead with our plan, or we will part ways as you choose another course of action.

 

The cost for these two meetings and the time I spend preparing your plan of care is a non-refundable $200, payable up front. If you choose to work with me, I will deduct the full amount from my fee. 

 

Alternatively, I offer a free 15-minute phone consultation to answer your questions regarding what I do and how I do it. This option is recommended only if you are in the information-seeking stage.

• Prepare/review legal documents, e.g. advance directive, living will, DNR, last will & testament, etc.

• Companionship/relief visits

• Support for medical aid in dying

• Post-death guidance and/or planning

• Bereavement home visits

• Written obituary

• Memorial service/funeral officiation
